For more than 4 decades, Horizons Unlimited of San Francisco, Inc. has provided Substance Use Disorder (SUD) Outpatient Treatment services to adolescents funded through the Department of Public Health. It has been an honor and privilege to work alongside many of you and the youth you have referred to our services and entrusted in our care.
While Horizons will continue to offer a range of SUD services from prevention and early intervention to outpatient treatment, we will no longer offer Drug Medi-Cal (DMC) SUD outpatient treatment services through the Department of Public Health effective December 31, 2024.
Please read our full letter for more information about this transition and how you can continue to refer clients in need of these and other critical services.

Dear Beloved Community Partner,
I hope this message finds you well and in the spirit of joy as we close out another beautiful calendar year with anticipation of what 2025 will bring.
I am writing to express my heartfelt gratitude for the trust you and your staff have placed in us as your longstanding Substance Use Disorder (SUD) Outpatient Treatment provider for adolescents. It has been our privilege to support the young people you have referred to our services as they embark on their journey of healing and recovery, and we are deeply committed to ensuring that they will continue to feel supported and work towards their recovery goals.
While we have been honored to work with you and your team, I must inform you that effective December 31, 2024, we will no longer offer Drug Medi-Cal (DMC) SUD Outpatient Treatment services through the San Francisco Department of Public Health. As your trusted provider, we are dedicated to ensuring that every young person in need of these critical services can access them easily, and with care and compassion. You and your staff can continue to refer Medi-Cal clients to the San Francisco Department of Public Health funded provider to access their DMC and other health care services. During this transition, we will continue to be a conduit for youth to enter treatment as well as continue our partnership with the Department of Public Health to offer SUD prevention and mental health and wellness services.
While we will no longer offer DMC SUD services funded through the Department of Public Health, Horizons, as a California Department of Health Care Services certified Alcohol and Other Drug (AOD) provider, will continue to offer a range of SUD services including prevention, early intervention and outpatient treatment that youth can access. Beginning January 6, 2025, you can refer non-Medi-Cal clients ages 12-24 to Horizons through our website.
Horizons also has an array of other services including mental health therapy, case management and wellness groups/activities, employment, arts/multimedia, and gender-affirming programs that we would love to share more about with you.
